How do I change the company address or phone number for my motor carrier operating authority (MC number)?
There are three ways to submit your address (or phone number/email address) change request:

Online: Address changes can be requested via the Licensing & Insurance (L&I) system. Select “Filing a change of address” and click “Continue.” Please note that you must have a credit card and USDOT PIN to submit the request

By email: You can complete and sign the MCSA-5889 Motor Carrier Records Change Form and submit it by uploading it via our web form. You will receive a tracking number in a confirmation email.

Mail: You may also mail the form to:

You also can fax your request form to 202-366-3477. Please do not fax it more than once, as that will delay the process. Your completed form must contain:
