I want to inactivate/deactivate my USDOT number. Can I submit the request online?

No, you can’t deactivate online or by phone. To close your USDOT number record you MUST mail, fax or upload via our web form an updated and signed MCS-150 (Motor Carriers) or MCS-150B (Hazmat Carriers) form, and check the box “out of business” (out of “motor carrier” operations, even if the company is still in business) in the “reason for filing” section.

Items 1-16, 30 and 31 on the MCS-150 must be completed or your form will be rejected.

If the owner has died, a death certificate needs to be included in the submission for us to be able to process it. 

You can submit the MCS-150/150B form to FMCSA in one of these ways:
  • Upload your updated and signed form via our web form (theis is the fastest way, and you will receive a tracking number)
  • By fax: You can fax the MCS-150 form to: 202-366-3477 or 606-330-3802
  • By mail: Send the updated and signed MCS-150 form to:
    US Department of Transportation
    Federal Motor Carrier Safety Administration
    Office of Registration and Safety Information (MC-RS)
    Room W65-206

    Washington, D.C. 20590
